It makes sense to me to gather some kind of history from each player before the start of the season, but I'm concerned about being too intrusive. What information should I collect?

• b. medical records that list every medical problem the athlete has experienced, including dates of surgeries and the names and contact information of the surgeons
• c. an emergency information card that contains names and contact information for the player's parent/guardian and physician, including information on any preexisting medical problems that may influence the athlete's treatment
• d. a parent/guardian consent form that gives you permission to provide first aid care to a minor
• e. a parent/guardian consent form that permits you to transport an injured player to the hospital
