Permissions Information Checklist
If you wish permission to use Human Kinetics materials within other publications,
send a request to HK via e-mail
(permissions@hkusa.com), fax 217-351-2674, or regular postal mail to Human Kinetics,
Permissions Editor, P.O. Box 5076, Champaign, IL 61825-5076. You may also submit your
information using our online permissions
request form.
Include the following information in your request:
How you will use the HK material
- If the material will be published by a publisher other than HK, provide the title
of the work to be released, its author(s) or editor(s), the publisher's name, the
estimated release date, the number of pages, and the binding (if known). If the
material is for nonclassroom use, indicate how many copies of it you wish to make.
For course packets, contact the Academic Permissions Service of the Copyright
Clearance Center. APS may be reached by calling 508-750-8400, faxing 508-750-4470,
or by writing 222 Rosewood Drive, Danvers, MA 01923.
- Full title of the original HK book or journal; full title of the chapter or section,
if applicable; last name and initials of the author(s) and/or editor(s).
- Volume and issue number of the journal or the edition number of the book.
- Exact page numbers as they appear in the HK publication of the material you wish
to use.
- If the request is for a table or figure, include table or figure number(s), in addition
to the page number(s) in the HK publication.
- Copyright holder and the year (HK serves as copyright agent for all HK publications,
regardless of copyright holder).
- If the material is changed in any way, a copy of the original material as it appears
in the HK publication and a copy showing your adaptation, summary, or other proposed
use.
Additional Information
- To direct your request to the correct publisher, you should check on the copyright page
or within the text for the original source of the material. The material you wish to use
may be credited to a source other than HK. If so, you should apply to that source for
permission, not to HK.
- If you submit a request to include an entire HK article or chapter in a publication by
a different publisher, and we grant permission for the use, we may require that you also
contact the author of the material for additional permission.
- Under no circumstances will we permit more than 10 percent of a book or 40 percent of
a journal to be reproduced.
- You may be required to pay a permission fee.
